The Short Version

  • CPT 92507 is deleted on January 1, 2027. Keep using it for dates of service through December 31, 2026.
  • It is replaced by 10 new timed speech therapy CPT codes, 92654–92663: five clinical categories, each with a base code for the initial 30 minutes and an add-on code for each additional 15 minutes.
  • Report the base code at 16 minutes of direct treatment; the first add-on at 38 minutes, the second at 53.
  • 92508 (group treatment) stays as a stand-alone, untimed code.
  • Medicare starts January 1, 2027; Medicaid and commercial payers may adopt the new codes on their own timelines. Use the 2027 SLP code calculator to check any session.

For decades, most individual speech-language pathology treatment was billed with one untimed code: 92507. That ends on January 1, 2027. Here is what replaces it, how the new speech therapy CPT codes work, and what your clinic should change before the first claim of 2027.

What Replaces CPT 92507 in 2027?

Starting January 1, 2027, CPT 92507 is replaced by 10 new speech-language pathology treatment codes, 92654 through 92663. They describe individual treatment in five clinical categories: fluency; speech sound production; language comprehension and expression; combined speech sound production and language; and voice, upper airway dysfunction and/or resonance. Each category has a base code for the initial 30 minutes of direct, one-on-one treatment and an add-on code for each additional 15 minutes.

The change came out of a 2024 CMS and AMA review that flagged fast growth in Medicare billing for 92507. ASHA worked with the AMA CPT Editorial Panel on a replacement that describes the service more precisely and recognizes that treatment time varies. The AMA published the official code numbers in the CPT 2027 code set in September 2026, and CMS proposed Medicare payment for them in the CY 2027 Physician Fee Schedule proposed rule.

Don’t switch early. The new codes apply to dates of service on or after January 1, 2027. Bill 92507 through December 31, 2026 unless a payer tells you otherwise.

The New Speech Therapy CPT Codes for 2027: All 10 Codes

The 2027 speech-language pathology treatment codes come in five base-and-add-on pairs: 92654/92655 (fluency), 92656/92657 (speech sound production), 92658/92659 (language comprehension and expression), 92660/92661 (combined speech sound and language) and 92662/92663 (voice, upper airway dysfunction and/or resonance).

Clinical categoryBase code: initial 30 minutesAdd-on: each additional 15 minutesExamples treated
Fluency disorders92654+92655Stuttering, cluttering
Speech sound production disorders92656+92657Articulation, phonological process, apraxia, dysarthria
Language comprehension and expression disorders92658+92659Receptive and expressive language, aphasia, late language emergence
Combined speech sound production and language92660+92661A child with articulation and language disorders; an adult with dysarthria and aphasia
Voice, upper airway dysfunction and/or resonance92662+92663Vocal nodules, spasmodic dysphonia, vocal fold paralysis, inducible laryngeal obstruction
Group treatment (2 or more)92508, untimed, stand-aloneAny speech, language, voice or communication disorder in a group

All individual codes describe direct, one-on-one patient contact. Add-on codes (+) are always reported with their own base code. Confirm full descriptors in the AMA CPT® 2027 code set.

CPT 92507 Crosswalk: Which New Code Fits the Session?

There is no one-to-one replacement for 92507. Choose the new code by the disorder treated in the session and the direct treatment time, not by the diagnosis alone.

Session in 2026 (billed as 92507)2027 code(s)Why
30 minutes of stuttering treatment92654 × 1Fluency, 16–37 minutes
45 minutes of articulation therapy92656 × 1 + 92657 × 1Speech sound production, 38–52 minutes
55 minutes of aphasia treatment92658 × 1 + 92659 × 2Language, 53–67 minutes
40 minutes on a child’s articulation and expressive language92660 × 1 + 92661 × 1Both speech sound and language in one session → combined code
30 minutes of voice therapy for vocal nodules92662 × 1Voice, 16–37 minutes
12 minutes of individual treatmentNot separately reportableUnder 16 minutes for a base code

How the New Timed SLP Codes Work: 30-Minute Base + 15-Minute Add-On

Under the general CPT midpoint rule, report the base code after 16 minutes of direct treatment, report the first add-on once the full 30 minutes plus at least 8 more minutes are reached (38 minutes), and each further add-on at 53, 68 and 83 minutes. Under 16 minutes, the service isn’t reported separately.

Direct one-on-one treatment timeWhat to report
Less than 16 minutesNot reported separately
16–37 minutesBase code × 1
38–52 minutesBase code × 1 + add-on × 1
53–67 minutesBase code × 1 + add-on × 2
68–82 minutesBase code × 1 + add-on × 3

Two rules matter as much as the thresholds. Time spent on an untimed service, or on a different timed service, can’t be counted toward these codes. And the same minutes can’t be counted twice when you treat two categories in one visit. Payers and CMS can set different rules, so check each payer’s policy. For how Medicare counts timed units in other therapy codes, see our Medicare 8-minute rule guide and timed vs untimed CPT codes.

Coding Rules for the New SLP Codes: What You Can and Can’t Combine

  • Use the combined code when you treat both speech sound and language. Don’t report 92656/92657 with 92658/92659; report 92660/92661 instead. Don’t report the combined codes with either separate pair.
  • Add-ons never stand alone. 92655, 92657, 92659, 92661 and 92663 are reported only with their own base code.
  • Different categories on the same day are allowed when each service is distinct, medically necessary, separately timed and meets its own time threshold, for example speech sound treatment and voice treatment.
  • Other services can be added when separate and distinct, such as swallowing treatment (92526), with no shared minutes.
  • Not with adaptive behavior treatment: the new codes aren’t reported with 97153 or 97155, and 92508 isn’t reported with 97154 or 97158.
  • Watch for edits: NCCI and medically unlikely edit (MUE) limits for the new codes are expected in late 2026.

Does CPT 92508 Change in 2027?

CPT 92508 stays as an untimed, stand-alone code for group treatment of speech, language, voice, communication and/or auditory processing disorders with two or more individuals, under revised CPT instructions. There is no minimum time requirement.

How Much Will Medicare Pay for the New Speech Therapy Codes?

In the CY 2027 proposed rule, CMS accepted the recommended values for all 10 codes. ASHA-reported proposed national payments range from about $49.92 to $71.59 for the 30-minute base codes and about $22.99 to $27.91 for each 15-minute add-on. These are proposed figures; final rates come with the CY 2027 final rule.

CodeServiceProposed national payment
92654 / +92655Fluency: base / add-on$54.55 / $23.97
92656 / +92657Speech sound production: base / add-on$66.01 / $23.97
92658 / +92659Language: base / add-on$49.92 / $22.99
92660 / +92661Combined speech sound and language: base / add-on$71.59 / $27.91
92662 / +92663Voice, upper airway and/or resonance: base / add-on$54.84 / $23.97

Proposed CY 2027 national non-facility amounts as reported by ASHA and state associations, before geographic adjustment. Final values may differ.

Two proposals to watch in the final rule

  • Multiple procedure payment reduction (MPPR): CMS proposed designating the codes “always therapy” and applying MPPR to the five 30-minute base codes, not the add-ons. ASHA has opposed this.
  • GSLPP pediatric code: CMS proposed a Medicare-only HCPCS code, GSLPP, for individual pediatric speech-language pathology treatment (proposed national payment about $66.34). ASHA asked CMS to withdraw it. It is not final.

Will Medicaid and Commercial Payers Use the New SLP Codes?

Medicare uses the new codes for dates of service starting January 1, 2027. State Medicaid programs and commercial plans decide if and when to adopt them, set their own payment rates, and may keep accepting 92507 until their systems and fee schedules are updated.

Some state associations are already tracking payers. In Pennsylvania, for example, PSHA reports that Medicaid changes won’t take effect before March 2027 at the earliest. For each payer you bill, confirm the effective date, prior authorization and coverage rules, documentation expectations and the new fee schedule. Medicare Advantage plans may also need new authorizations if the code changes mid-episode.

How SLP Documentation Must Change for 2027

Under 92507, many notes didn’t record minutes. Under the new codes, every note must show which clinical category was treated, the goals addressed and the direct one-on-one minutes for each service, with separate time when more than one category is treated.

  • Record start and stop times, or exact direct minutes, for each category treated.
  • Link each goal to its clinical category (fluency, speech sound, language, combined, voice).
  • Keep minutes for untimed services, such as evaluations and group, separate.
  • Document medical necessity for each category when more than one is billed.
  • Update plans of care and goals so categories are clear before January.

For general documentation rules, see our therapy documentation compliance guide.

2027 SLP Coding Readiness Checklist

  1. Train SLPs, billers and front-desk staff on the five categories and time thresholds.
  2. Ask your EHR and billing vendor when the new codes, add-on logic and edits will be live.
  3. Update note templates to capture category and direct minutes.
  4. Update your charge master and fee schedule for all 10 codes.
  5. Contact every payer about effective dates, authorizations and rates.
  6. Review contracts that pay a per-visit rate for 92507.
  7. Plan for authorizations that span December 2026 into January 2027.
  8. Check the CY 2027 final rule for final payment, MPPR and GSLPP decisions.
  9. Audit the first weeks of 2027 claims for code and unit errors.

New Speech Therapy CPT Codes 2027: FAQs

Starting January 1, 2027, CPT 92507 is replaced by 10 timed speech-language pathology treatment codes, 92654 through 92663, in five clinical categories: fluency, speech sound production, language comprehension and expression, combined speech sound and language, and voice, upper airway dysfunction and/or resonance.

The new codes take effect for dates of service on or after January 1, 2027. CPT 92507 remains in effect through December 31, 2026. Some payers may adopt the new codes later.

No. ASHA advises clinicians not to report the new codes before their effective date. Keep billing 92507 for 2026 dates of service unless a payer instructs otherwise.

Under the general CPT midpoint rule, report the 30-minute base code after 16 minutes of direct, one-on-one treatment. Report the first 15-minute add-on at 38 minutes, the second at 53 minutes and the third at 68 minutes.

CPT 92508 stays as an untimed, stand-alone code for group treatment of two or more individuals, with revised CPT instructions and no minimum time requirement.

No. When a session treats both a speech sound production disorder and a language comprehension and expression disorder, report the combined codes 92660 and 92661 instead of the separate speech sound and language codes.

Yes, when each service is distinct, medically necessary, separately timed and meets its own time requirement, for example speech sound treatment and voice treatment. The same minutes can’t be counted toward both.

92654 is treatment of a fluency disorder, such as stuttering or cluttering, for the initial 30 minutes of direct one-on-one contact. 92655 is the add-on for each additional 15 minutes and is reported only with 92654.

92658 is treatment of a language comprehension and expression disorder for the initial 30 minutes of direct one-on-one contact. 92659 is the add-on for each additional 15 minutes and is reported only with 92658.

In the CY 2027 proposed rule, proposed national payments range from about $49.92 to $71.59 for the 30-minute base codes and about $22.99 to $27.91 for each 15-minute add-on, before geographic adjustment. Final rates come with the CY 2027 final rule.

Medicaid programs and commercial plans decide if and when to adopt the new codes and set their own rates. Some may keep accepting 92507 for a time, so confirm each payer’s effective date and billing rules.

GSLPP is a Medicare-only HCPCS code CMS proposed for individual pediatric speech-language pathology treatment, with a proposed national payment of about $66.34. ASHA asked CMS to withdraw it, and it is not final until the CY 2027 final rule.

CMS proposed designating the new codes as always therapy and applying the multiple procedure payment reduction to the 30-minute base codes, not the add-on codes. ASHA has opposed this; the final rule will decide.

Notes should record the clinical category treated, the goals addressed and the direct one-on-one minutes for each service, with separate time when more than one category is treated, because the new codes are timed.
